There are three ways of submitting an application:

| STEP 1 |
| You are required to have Adobe Acrobat Reader version 7.0 and above installed on your PC in order to fill these forms. |
| STEP 2 |
| Click on these application forms & fill up your details. |
|
| International students will need to submit the following medical form along with your application. |
| STEP 3 |
| Once completed, the application forms can be submitted through your email application as described in email guide form. |

Electronic application forms use the Adobe PDF Forms technology, which are essentially fillable PDF documents.

| If you wish to have a set of application materials sent to you, please send a self-addressed 30x25 cm stamped (RM1) envelope to MUST OR |
| These forms can also be obtained by visiting our Kelana Jaya campus. |
| STEP 2 |
| Print the application forms and manually complete these forms. |

| STEP 3 |
Completed application forms have to submitted to:

Note: MUST understand that it can be a difficult process to gather the evaluation forms and so encourages you to submit your application while you are in the process of getting your evaluators to fill up the forms. It will not affect your application process.
Follow the instructions which are on pages 1 and 2 of the application form and submit the application to MUST.
The above materials may be photocopied.
