Application Procedure

There are two ways for submitting your applications:-

  1. Electronic Application Submission

    • Electronic application forms use the Adobe PDF Forms technology, which are essentially fillable PDF documents. You are required to have Adobe Acrobat Reader version 7.0 and above installed on your PC in order to fill these forms. You can download the FREE Adobe Reader software from this site.

    • If you have Adobe Acrobat 7.0 (and above) installed, please proceed to download the appropriate application form from the following links:

    • Once completed, the application forms can be submitted through your email application as described in this guide.


  2. Paper Application Submission

    • The application forms can be downloaded for printing using the links mentioned below:

    • Alternatively, if you wish to have a set of application materials sent to you, please send a self-addressed 30x25 cm stamped (RM1/-) envelope to MUST.

    • These forms can also be obtained by visiting our Kelana Jaya campus. View our location map.

    • Completed application forms have to submitted to:

      47301 Petaling Jaya,
      Selangor Darul Ehsan.

(Note: MUST understands that it can be a difficult process to gather the evaluation forms and so encourages you to submit your application while you are in the process of getting your evaluators to fill up the forms. It will not affect your application process).

Follow the instructions which are on pages 1 and 2 of the application form and submit the application to MUST.
